POSITION SUMMARY: Lead Case Manager participates in all programs and activities for staff and volunteers and supervises other case managers and staff who conduct case management under the direction of the Emergency Shelter Manager. Lead Case Manager empowers survivors by following the agencies mission and goals. The Lead Case Manager supports the Program Manager to ensure the success of the Emergency Shelter Programs. PRIMARY POSITION PRINCIPLES: · The Lead Case Manager provides leadership and guidance to the program's staff and supports the Program Manager with various programmatic tasks to ensure its upmost quality· Providing support and guidance to colleagues regarding service delivery best practices including making effective and appropriate referrals and providing on-going support that helps participants meet their goals· Assisting with program administration, the training of staff and representation of the program internally and externally· Ensuring that evidence-based practices are utilized through example and by providing training· Providing on-going case management support to assess progress and adjust as necessary· Coordinate the integration of social services/ case management functions into the participant's planning process· Conduct regularly review of all participants file from all the program· Keeps detailed records· Help participants work through urgent needs. Enable clients to take responsibility for their recovery, acting as an advocate and a facilitator. Work in partnership with participants· Relay all information to Shelter Manager on a weekly basis DUTIES AND RESPONSIBILITIES: · Admit, process, and provide an orientation to new clients utilizing agency intake procedures· Carry a caseload of 2-9 participants assigned by the Shelter Manager, providing full case management, advocacy, and individual counseling· Must meet funding source requirements for case management, counseling, and group session per participant· Extensive knowledge of common DV funding sources ( i.e., CalWorks, OES, CSBG, ARP)· Group counseling sessions and child respite is needed· Under the direction of the Shelter Manager assign a case load to other case managers or staff who performs case management functions· Make referrals and assist participants with their planning· Maintain documentation for all services provided· Answer hotline calls and give appropriate referrals· Case consultations in all case management meetings with staff and supervisors.· Provide transportation for clients as needed· Documentation for assigned cases· Collaborate with the staff and hold regular meetings, discussing current cases in order to provide the best possible service to clients as a cohesive team· Responsible for ensuring the cleanliness, orderliness, safety and sanitation of the shelter through supervision of residents in their household duties· Represent the agency at community meetings and special events as assigned, such as councils, chambers of commerce, etc.· Along with Su Casa staff, assist in conducting orientations to new volunteers/interns, as requested· Responsible for completing and submitting monthly and quarterly reports· Additional duties and special projects may be assigned as necessary MINIMUM QUALIFICATIONS: 1. Bachelor's degree in human services or related field2. Certification in the 40-hour domestic violence victim services training required by the State of California or willing to obtain within first 3 months of employment3. Bilingual required in English/Spanish4. At least two to three years of experience of working as a Case Manager5. At least one year of experience in a leadership position6. Excellent written and oral communications skills7. Possess basic computer knowledge and skills8. Ability to work as a team member and have good interpersonal skills9. Ability to work independently with minimal supervision10. Vehicle required with valid CA driver's license and auto liability insurance11. Has not been a client of Su Casa or any other DV Shelter nor been in a domestic violent relationship for one year12. Clear state and Department of Justice criminal background check13. Sensitive and supportive to individuals and families in crisis14. Able to work as a team member. Exemplary organization skills, good interpersonal skills, strong leadership qualities, and good judgment15. Extensive knowledge in DV grants and funding sources, billing and documentation This job description is not intended to be all-inclusive.
