Title: Clinical Transport & Immunization Nurse Company: Children's Habilitation Center Website: https://childhabcenter.com/ Classification: Full Time; Non-Exempt Reports to: Director of Nursing/Administrator Supervisor Responsibilities: No Location: 121 W. 154th St in Harvey, IL Shift: Varies depending on the needs of our residents Purpose: The Clinical Transport/Immunization Nurse works to provide high quality acute and chronic care nursing to our pediatric and adult population by professionally communicating with specialists, assessing problems and anticipating potential problems. Responsibilities: Schedules appointments for routine Doctor visits, diagnostics testing, and procedures as ordered by Children's Habilitation Center Medical Director, Nurse Practitioner or Physician Assistant in a timely manner. Schedules transportation to and from appointments, and complete the necessary documentation as requested by transportation Co./ provider (ex: POS & LON & MCO forms). Ensures all documentation is complete for billing purposes. Schedules in-house clinics for Eye, Podiatry and Dental evaluations and make rounds with providers Accompanies residents on clinic transport via ambulance or ensures resident is accompanied by appropriate CHC staff Maintains a flexible work schedule according to resident needs, including overnights for sleep study Communicates with CHC MD and clinicians, as it relates to resident medical conditions and next steps. Ensures clear, accurate information is relayed during clinic visits including lab result, diagnostic reports and completes documentation on portable I-Pad into electronic health record while on visit. Writes Verbal and/or phone orders from CHC MD, APN and/ or PA Liaison between CHC medical providers and outside consultants as it relates to residents' medical needs Qualifications: Active IL State Registered Nursing (RN) license or Licensed Practical Nurse (LPN) Minimum of two (2) years of immunization/vaccination experience in a hospital or clinical setting American Health Association approved CPR certification Ability to pass criminal background check and drug testing Experience working with various socioeconomic populations is a plus. Must be able to lift, push, pull or carry a minimum of 50 lbs.
