SUMMARY: As a part of a multidisciplinary team, the Case Manager and Outreach Worker is responsible for assessing the needs of patients and determining what existing programs those patients qualify for and developing individualized plans. in accordance with CSMD policies and procedures, professional standards of practice, and requirements of federal, state and local regulatory and/or licensing agencies/organizations This position works in a clinical setting directly with patients from vulnerable populations to connect them with available support services. Patients typically include seniors, low income, uninsured/underinsured individuals and people with physical or developmental disabilities. PRIMARY RESPONSIBILITIES: Has knowledge and understanding of Employee Handbook. Carries out the meaning of the CSMD Mission and Vision statements. Practices a culture of safety to reduce or prevent risk of injury, claims, loss, or liability by utilizing the Risk Management and Infection Control Plan. Performs responsibilities with a high-quality standard to yield improved compliance, quality, and patient outcome measures by adhering to the Quality Improvement Plan. Is present and ready for work, as scheduled. Attends in-service trainings, departmental meetings, and community events (when requested). Works cohesively with team members. Identify and assess issues that patients are experiencing; and implement solutions such as relocation, housing assistance, living needs, education, employment assistance, etc. Consults with Chief Operations Officer and Operations Manager before decisions are made on questions of interpretation. Identify community resources and build collaborative partnerships. Build relationships based on trust and compassion. Partner with patients in developing to help patients meet their needs. Commit as the accountability and patient engagement success partner. Advocate for patients in community settings and with other agencies. Organizes work to track and follow through on any referrals, requested support services case management or other necessary information to help multidisciplinary care team meet the client needs. Build new relationships and maintain existing ones with current and future community entities that are providing services to consumers to promote health insurance coverage through the Federal Marketplace and Healthy Louisiana (Medicaid and CHIP). Provide application and renewal assistance and facilitate enrollment of eligible patients and community members in health insurance programs by providing fair, impartial, and accurate information. Maintain knowledge and expertise in eligibility, enrollment, and program specifications of the Federal Marketplace and other health coverage programs such as Healthy Louisiana (Medicaid and CHIP). Assist with the development and dissemination of marketing and promotional materials for outreach, application assistance, and education activities that are tailored to the health center service delivery area and target population. Provide referrals for consumers with questions, complaints, or grievances to any applicable office of health insurance consumer assistance or health insurance ombudsman, or any other appropriate state agency or agencies. Conduct monthly in-reach, outreach, and education activities to existing health center patients and community residents to promote awareness about coverage options available through the Federal Marketplace and Healthy Louisiana. Ensure the protection and security of a consumer's personal, confidential and identifiable information in a professional and responsible manner and carry out all measures to prevent from unauthorized disclosures. May be responsible for grant management support and / or budgetary tasks such as small purchases and credit card reconciliation with Finance. Prepares and presents monthly reports to supervisor outlining individual plans/goals/interventions (when requested) to meet the Centers overall productivity and performance improvement goals. Provides an effective communication link between patients and providers. Meets or exceeds organizational standards as it relates to improving client's health literacy. Attends required organizational meetings and participation on internal committees. Participates in in-service training as required or requested. Utilizes technology to create a more efficient and effective way of doing business. Actively participates in professional association activities which aligns with the values and mission of CSMD. Maintains privacy and confidentiality of workplace information according to the policies and procedures of organization and HIPAA guidelines. Performs other duties as assigned by supervisor and/or the Chief Medical Officer. SUPERVISES: None. TRAINING: Relias CareSouth Onboarding Relias Track Non-Clinical PERFORMANCE MANAGEMENT EVALUATION TYPE: Administrative and Programmatic REQUIREMENTS: EDUCATION: Bachelor of Science in Social Work from an accredited college or university. TRAINING AND EXPERIENCE: One to two years of experience working as a Social Worker or similar position in patient case management. Effective communication skills: both oral and written. Ability to work with others or within a team and interpret a variety of instructions furnished in written, oral, diagram or schedule form. JOB KNOWLEDGE: Ability to organize, prioritize and work independently as well as schedule and produce work in a timely manner. Experience and ability to effectively utilize personal computers, various hardware and software packages. PHYSICAL REQUIREMENTS: Visual acuity - always Hand - eye coordination - always Lifting approximately 10-15 lbs., Pushing, Pulling - sometimes Stooping, Bending, Sitting, Standing - sometimes. Walking- frequent; short distances Must be capable of standing on a step stool and reaching above head and shoulder area.
