About the Opportunity Reporting to the Manager of Seniors (Western Zone), or designate, the incumbent is a member of the Seniors LINCS Team and participates in the planning, development and provision of Social Work services to community-based seniors to enhance their independence and function at home. The Social Worker works to full scope of practice as an independent practitioner within the team. This includes supporting the development and integration of seniors’ health.
