St. Gabriel's Hospice & Palliative Care is currently seeking a RN Case Manager to join our team in San Antonio! About Us St. Gabriel’s Hospice is a compassionate and patient-centered organization dedicated to providing exceptional end-of-life care. Our team of healthcare professionals is committed to supporting patients and their families during this critical time. Position Overview We are seeking a PRN (as-needed) Registered Nurse (RN) Case Manager to join our dynamic team. As a Case Manager, you will play a pivotal role in coordinating and delivering comprehensive hospice care to our patients. Your compassion, clinical expertise, and communication skills will make a significant impact on the lives of those we serve. Job Qualifications Education: Must be a graduate of an accredited Nursing School Licensure: Current state license as a Registered Nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact. Current driver’s license. Experience: Two (2) years experience as a registered nurse in a clinical care setting. Hospice experience preferred. Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life. Must read, write and comprehend English. Transportation: Reliable transportation and valid auto liability insurance. Responsibilities Conduct initial assessments and ongoing evaluations of patients’ needs Develop and implement individualized care plans Collaborate with interdisciplinary teams to ensure holistic care Provide education and emotional support to patients and families Coordinate services, including home visits and referrals Maintain accurate documentation and records Participate in quality improvement initiatives Join our team at St. Gabriel’s Hospice and contribute to our mission of providing comfort, dignity, and compassion to those in need.
