Profession: Physical Therapist (PT) Location: Kayenta AZ Specialty: Physical Therapist (PT) Unit: Gross Pay: TBD Schedule: Days - 5 x 8, 07:00:00-15:00:00, 8.00-5 Guaranteed Hours: 40 Openings: 1 Start Date: 07/01/2024 End Date: 06/28/2025 Assignment Length: 52 Experience Required: 1 year JOB ID: Laboredge59660069 Other Info: ID: 59660069 Shift: 8 HR Days 2 08:00-17:00 Description: Job Specific Notes: Two students have Physical Therapy needs currently noted on their Individual Education Plan.Two students who are currently in the Exceptional Education program have concerns that may beresolved with Physical therapy and need to be screened and may require further evaluation.Further, the contractor will be involved in a school-wide Child Find and Screening for allkindergarten studentsTHE CONTRACTOR WILL:1. Review student s medical records, physician referral, child study team referral, parentrequest and/or school psychologist evaluation recommendation for further testing;2. Perform evaluation procedures to establish performance guidelines;3. Provide formal written comprehensive test results of student assessment 30 days fromdate of evaluation;4. Plan, prepare, and establish appropriate written Individual Education Plan goals andobjectives;5. Attend student s scheduled IEP meeting;6. Administer and implement physical therapy/occupational therapy program during 12 sitevisits (scheduled by the contractor) occurring throughout the period of performance.7. Re-assess and revise program, as necessary, as status of student changes;8. Instruct and teach students, families and teachers as indicated on IEP;9. Make referrals to other health professionals and integrate physical/occupational therapyprogram with other aspects of health care;10. Confer with physician and other health practitioners to obtain information and makerecommendations and revisions in treatment program;11. Supervise and train education technicians and/or certified teachers inoccupational/physical therapy assistance to be carried out when the occupational/physicaltherapist is not at the school;12. Provide weekly on site services during the school year; as warranted by student need;13. Provide a progress report for individual students to coincide with the school s gradingperiods;14. Attend a contract provider s meeting with the Agency Special Education Office15. Provide a copy of their state certification to Kayenta Boarding School within two weeksof the beginning date of service;16. Inform the school of the specific provider who must have an approved securitybackground investigation clearance approval.17.
